Submittal documents contain crucial information about the document itself, the preparer, the review process, and the dates items were sent and received. When contractors send a submittal package, they typically include a transmittal cover sheet that lists all the submittals included, allowing the recipient to verify that nothing is missing.

1. Project Name
2. Spec Section: The specification number corresponding to the submittal or package
3 Submittal Number: A unique identifier for the submitted item
4. Revision or Version Number: Used when creating new versions in response to a revision request
5. Package Number: Indicates the number of the package if the submittal is part of one
6. Submittal Manager: Name and contact information of the person managing the documents, usually a project manager or project engineer from the General Contractor
7. Submission Date: The date the submittal was sent
8. Received Date: The date the submittal was received
9. Approver(s): Names and contact information of those responsible for review and approval
10.Response: Typically includes preset options such as Approved, Approved as Noted, Revise and Resubmit, and Rejected
11.Notes/Remarks: Space for the reviewer to provide details about requested changes or revisions.
